Follow the link to the ESET website for a link to their knowledge base article for what you can do if you have already installed Vista SP2. Not an ideal solution (uninstall v4.x and install 3.x). Don't know if you can just uninstall Vista SP2 to fix the problem as article does not say and I am not willing to test.

An incompatibily between Service Pack 2 and the 4.0 versions of ESET Smart Security and ESET NOD32 Antivirus can prevent the proper functioning of real-time file-system protection. The ESET icon next to the system clock will turn red if you are experiencing this issue, notifying you that maximum protection is not ensured. An updated build for version 4.0 ESET security products is currently in development and will be released as soon as possible.
